This program provides health assessments and health education programs for individuals that attend any of the Cafes or Enrichment Centers. Disease prevention and health promotion services provide educational opportunities and information about prevention, treatment, and reduction of the effects of chronic disabling conditions to adults age 60 and older.

Information about promoting healthy lifestyles is also provided. The goal of the program is to empower older adults to improve health and prevent disease by taking more control over their own health through lifestyle and behavioral changes.

Depression screenings / counseling are available to eligible individuals age 60 and older, and are performed when referred by the care management staff. Subjects can include the following:
